Surplus Request
Submitting a surplus request for lab equipment must be accompanied by an EH&S decontamination form. Failure to include proper documentation will result in a delay disposing of the equipment.

Please decontaminate equipment properly before submitting this request and submit the form with EH&S.

Please also attach any appropriate refrigerant removal forms to this request at the bottom.
