EPI Seminar Request Form

We appreciate your interest in hosting a seminar within the EPI Seminar Series. We advise you read the EPI Seminar Event Policy prior to completion of this form to ensure you are familiar with standards and expectations. The entire form must be completed in order to invite a speaker into the EPI Seminar Series; please allocate all required information to proceed. All requests are subject to review and approval by the administration team. Completing a form does not guarantee the request will be fulfilled. Please allow 3-5 business days for a response.
